Registered Nurses

Registered nurses from Greenbelt Home Care provide skilled home care for people who have an illness or have been in the hospital.  

They keep your physician updated on your recovery, and perform skilled tasks such as drawing blood for lab tests, giving medicine by injection, caring for a wound, or teaching you and your family about your medications & treatments.  Our nurses help you return to your independent lifestyle, and are knowledgeable of resources in our community that will help you recover and stay well.  They can also make referrals to several different agencies in the community.  

Nursing services are funded by Medicare, Medicaid, Veterans Affairs, private insurance, grants, and the Hardin County Board of Supervisors. 

A Greenbelt Home Care nurse is always on call.

  • Skilled Nursing Services:
    • Evaluate client's nursing needs.
    • Provide any services requiring skilled nursing such as medication set-up, injections, blood draws, dressing changes, diabetic teaching and prefilling of insulin syringes, etc. 
    • Coordinate all services required by the client.
    • Assist frail and elderly in long-term planning to stay in home or nursing home placement.
    • Client and family health education.
    • Supervise Home Care Aides providing service.

Home Care Aides

Our trained Home Care Aides are available to assist you with a variety of needs.  They offer personal care services such as bathing, skin & hair care, and foot care.  Home Care Aides can also assist with budgeting, meal planning & preparation, grocery shopping, household chores such as vacuuming and dusting, laundry, making the beds, and washing dishes. Home Care Aides are funded by Medicare, Medicaid, Veterans Affairs, private insurance, grants, and the Hardin County Board of Supervisors. 

  • Home Care Aide Services:
    • Provide personal care such as bathing, skin, and hair care.
    • Assistance with home management and light housekeeping.
    • Assistance with grocery shopping and laundry.
    • Assistance with meal planning and preparation.
    • Assistance with rehabilitation and exercise therapy.

Thrive by Five

Thrive by Five is a long term home visiting program offered to families in Hardin County.  The program is designed to share information and advocate for families.  The Thrive by Five program provides skills related to parenting, child development milestones, developing problem solving techniques, and strengthening family relationships.  This program is funded through the Iowa River Valley Early Childhood Area family support grant.  To qualify for the Thrive by Five program, families must be at or below 200% of the federal poverty level, have one or more children ages prenatal through five, and the family must reside in Hardin County.  

How to Make a Referral

Making a referral to Greenbelt Home Care is very easy. Requests for services may come from any person, including self, family, friends, neighbors, physicians, nurses, social workers, hospitals, or other community agencies.  All you have to do is call or fax Greenbelt Home Care and provide us with some general information and we can take it from there.
