RN Clinical Manager

The RN Clinical Manager is responsible for the overall operation of the disease and disability nursing program , recommends hiring of professional staff, markets the program, monitors the budgetary expenditures of the program and assures that records are maintained and data is collected, supervisions of nursing staff, teams as required, reviews and updates policy and procedures as needed. Must maintain competency on all regulatory and credentialing standards. 

QUALIFICATIONS:

· Must have a current RN license from the State of Iowa, BSN preferred.

· Home health experience preferred.

· Knowledge of Medicare/Medicaid regulatory and credentialing standards and criteria is required.

· Mandatory reporter for adult and child certified, BLS certified.

· Will act as a RN Case Manager by performing all duties when RN case managers are off due to illness, vacation, termination or shortage in staff. 


RN Maternal Child Health Coordinator

Greenbelt Home Care serves Hardin County and the surrounding townships.  The RN Maternal Child Health Coordinator is responsible for providing direct care, leadership and direction to agency personnel and members of the community to assure that the health care needs of families in Hardin County are met. 

QUALIFICATIONS:

  1. Current RN license from the State of Iowa.
  2. Two or more years of professional nursing experience required, medical/surgical, intensive care or inpatient rehabilitation experience desirable.
  3. Two years of public health or home care nursing where supervision of home health care aides has been a part of the duties of the position preferred.
  4. Must have a valid driver’s license and auto insurance.

Q. What is home health care?

A. Home care is health care delivered in an individual’s home.  Services may include skilled nursing, personal cares, physical, occupational and speech therapy, home maker services, and respite to a primary caregiver, among others.

Q. What are the advantages of home health care?

A. Home health care allows you or your loved one to live in the comfort of your own home, preventing or delaying the need to go to a hospital or long term care facility.  We believe that you or your loved one will be happier and healthier in your own home.

Q. How is Greenbelt Home Care different?

Q. How do I seek home care for myself or a loved one?

A. Call us at 641-939-8444 and we will take it from there.  We realize this is a stressful time for your family and we are here to make it simple.  We’ll contact your physician, verify your insurance, and visit you in the comfort of your own home.  Greenbelt Home Care is only a phone call away, and we’re here to serve you.

Q. Who pays for home care?

A. Home care is covered by most insurance plans, including Medicare and Medicaid. Our team of professionals will work with your insurance provider to verify your coverage. Other coverage options include Private Insurance, Veterans Administration, Waiver Programs, and private pay.

Q. Do you only work with seniors?

A. No. We serve all ages from prenatal mothers, to infants, up to seniors.

Q. I just had an accident and I was injured. Does home care make sense for me?

A. Absolutely. Our Therapists will assist you with rehab or any special equipment that you might need. Our Nurses will help with any medications or wounds, and our Home Health Aides can assist you with personal care needs. At Greenbelt Home Care, everyone works together as a team so you can go home.

Q. What about if I have a hip, knee, or shoulder replacement? Will Greenbelt Home Care work for me?

A. Yes. Just like we’d help if you had an accident, our Therapists will assist you with rehab or any special equipment that you might need. Our Nurses will help with any medications or wounds, and our Home Health Aides can assist you with personal care needs. Our team works together so you can go home after a joint replacement.

Q. How often will someone visit me?

A. The frequency of our visits will be determined by the services we’re providing. We’ll call ahead to confirm the visits, and make sure we properly identify ourselves when we arrive. Greenbelt Home Care doesn’t want there to be any surprises or uncertainty.
