RN Case Manager
The RN Case Manager is responsible for all home care services of patients on their caseload. We are currently looking to fill this position on a full-time basis. Greenbelt Home Care offers self-scheduling so there is the bonus of flexibility and independence. Also, we typically work from 8am to 5pm with limited on-call requirements.
1. Current RN license from the State of Iowa.
2. Two or more years of professional nursing experience, medical – surgical, intensive care or inpatient rehabilitation experience desirable.
3. Two years of public health or home care nursing preferred.
4. Analytical skills sufficient to identify and plan for the appropriate caregiver to provide timely home care services, meeting state and federal, third party payer and professional standards.
5. Must have the ability to function in a crisis situation and the interpersonal and communication skills necessary to assure that complex health care issues and peer and subordinate caregiver issues do not jeopardize the quality of patient care being delivered.
RN Clinical Manager
The Clinical Manager is accountable for the coordination and management of multidisciplinary care teams; Ensuring that patient care provided consistently maintaining the highest Home Health standards, continuity of care and services are delivered in the most efficient manner. The Clinical Manager position is on-call 24/7 to comply with Medicare regulations. The Clinical Manager is also responsible for filling in for RN Case Managers in the event they are ill, on vacation, or if there is a shortage in staff.
PRN Respite Provider

Greenbelt Home Care currently seeks PRN(as needed) Respite Providers to provide respite care services in Hardin County and surround areas. The purpose of respite care is to give the parent or primary caregiver a break from the daily responsibilities of caring for a child or adult with a disability.
Respite Providers have flexibility with the ability to make their own schedule around the family's needs.
Current Need: Varied hours as scheduled/needed.
This position provides all care required to a child or adult with a disability while their parent or guardian is absent, while maintaining a safe and nurturing home-based environment.
The Respite Provider position will not have any guaranteed or set hours. There may be extended periods of time with no hours being worked. In many situations a respite provider may work a few hours one day and then not work again for several weeks or months. All hours worked by a Respite Provider are at the request of the family and respite is often provided in the afternoons, evening and on weekends.
Qualified applicants will have a high school diploma or GED. Experience working with people with disabilities preferred, but not required.
This position requires a valid driver's license, auto insurance, a good driving record and the ability to pass an extensive background check including checks of the Dependent Adult and Child Abuse registries, Sex Offender registry and a criminal history check.
